Artistic Sports Physiotherapy Terms and Conditions

Patient Care

Artistic Sports Physiotherapy strives to provide you with an outstanding clinical experience. You will be treated with respect and acceptance towards all ages, genders and ethnicities. Artistic Sports Physiotherapy is LGBTQIA+ friendly.

Professional recommendations regarding your visit are based on the information provided by you and the objective findings from the physical examination. From this examination an informed consent is required to perform treatment.

Consent to Treatment

By agreeing, you give your consent for our therapists to carry out all required examinations, treatments, therapies, rehabilitation, and medical diagnostic procedures based on their professional training and assessment of your injury.

You acknowledge that you have the opportunity to discuss the nature and purpose of your physiotherapy care with your therapist before any treatment is provided. You have the right to refuse a treatment.

Fees and cancellations

  • Payment is required on the day of consultation
  • We accept cash, EFTPOS, credit cards and offer instant health rebates via HICAPS. However, in the event of electronic network failure, full payment is still required on the day of consultation.
  • Please be considerate of people waiting for an appointment and allow as much notice as possible to cancel your appointment. Please allow at least 24 hours to reschedule or cancel your appointment.
  • If you fail to attend your booked appointment without notice of cancellation, you will be subjected to pay the full fee of the consultation within 48hours of your appointment.
  • Failure to pay service fees will attract an administration fee.
  • As of 01/01/2025, the current service prices are: Initial consultations, Dance and Pointe Assessments and Extended consultations: $155, Standard Consultation and Vocal Unloading: $90.

 
Privacy Policy

Operation of Artistic Sports Physiotherapy includes adhering to a code of conduct under the Physiotherapy Board of Australia and adheres to the Australian Privacy Principles. The Australian Privacy Principles govern how personal information is managed, including its collection, use, disclosure, storage, access, and disposal. This ensures health care services are delivered appropriately, effectively and within an ethical framework.

There is a minimum set of information that is required to allow health care to proceed. Patients can expect that practitioners will hold personal information in confidence and understand that information may be used for the following;

  • Sharing information regarding your condition and treatment with your doctor, employer, case manager and other treatment providers.
  • For legal disclosures as required by a court of law.
  • You will be contacted via SMS and/or email to keep you up to date with the status of your appointment and to follow up on the progress of your condition.
  • To administer invoices.
  • Marketing: any subscription to marketing emails can be unsubscribed at any time by clicking the ‘unsubscribe’ option at the bottom of the email. 
  • You can gain access to your private information at any stage.

By engaging with our services, you consent to our privacy policy and the following;

  • I can give informed consent about my healthcare with knowledge and understanding of the benefits and risks involved.
  • I can access my personal treatment information on request.
  • I have read the above information and understand the reasons for collecting information and the ways in which this information may be used.
  • Artistic Sports Physiotherapy may contact me via SMS, email or telephone call for reasons related to my consultation.
  • I give permission for Artistic Sports Physio to collect data relating to my treatment, providing my privacy is ensured.
